How to fill out Annexure C
Annexure C is necessary only when one parent is absent and should be accompanied by a supporting document as outlined in the checklist, depending on your situation.
Should you require clarification, you can submit a questionnaire to VFS. If their response is generic, you can reply asking for more specific information, hoping to receive a tailored response for your case.
Additionally, this affidavit needs to be notarized, along with the other documents, as part of the renewal process.

Are Handwritten Annexure Accepted?
As per VFS instructions, it’s recommended to type out the information if you can use a PDF editor to fit in details. However, I’ve personally seen hundreds of Annexures and affidavit filled out by hand with pen. VFS has accepted handwritten annexures and affidavits.
Annexure C
Begin by entering your name – the full name of the parent applying for the passport and signing Annexure C, followed by your current USA address with zip code.
Question I: Provide the child’s name for whom you are applying for the new passport.
Question II: Indicate the parent’s name who is unavailable to sign any documents and whose consent has not been obtained.
Next select the applicable scenario from options A to G and mark the one that applies in your case.
You should cross out the green highlighted parts (see below screenshot) that do not apply to you, such as I/we, am/are, mother/father/parents/guardians, throughout the entire Annexure C form.


Question III and Question IV: Provide the child’s name for whom you are applying for the new passport.
Once you have filled out this form up to this point, you should wait to be in front of a notary to have your signature witnessed, along with indicating the place and time of signing this affidavit.

For notarization, I make it a practice to include the notary certificate wording directly on the same paper. I affix Acknowledgement Stamp or Jurat Stamp, along with my Notary Stamp, and conclude with my signature.
