What documents need to be Notarized for Minor Indian passport renewal in USA
When you apply for Indian passport renewal from the USA for Minor whose age is below 18 years, there are 6 mandatory documents which require Notarization.

Six Mandatory documents that require Notarization for Indian passport renewal for Minor in USA:
- Affidavit for change of appearance & Signature
- Color copy of the applicant’s Legal status in the USA
- Color Copy of Proof of Address
- Annexure D
- Both Parents Valid Legal Status in the USA

1. Affidavit for change of appearance and Signature
Yes, Even though Minor appearance and signature has not changed (mostly, which is not the case for minor ) you still need to submit this document.
Type or Hand write? Try to type out all information but if you not tech savvy like me hand writing should be fine too. I have seen hand written and typed both document accepted. I suggest do something only if you have explanation for it, like what to do if Minor’s long, big name don’t fit in and you don’t have other software to do adjustments?
Here’s what you fill out : I <Minor’s Full Name as per current passport (New name after change)>, Son/Daughter of <father’s full name as per Current passport > , resident of <Current USA address, exactly as per address proof>, and holder of passport No.<Expiring passport number> , issued on <DD/MMM/YYYY> at <Issue place exactly as per expiring passport>
How to decide if Minor’s appearance has changed or not? I have seen, for minor appearance and signature are mostly changed. Whatever you think, have or have not changed, both answers are correct.
Leave Minor’s signature, age, both parent signature date and place to fill in presence of Notary public.
Not to mention, With the glue stick the photo within the box (please check the photo specification).
2. Color copy of the applicant’s Legal status in the USA
If Minor have a Green card or Stamped Visa on minor’s passport like H4, get a color copy and front and back both side on same page for Green card
Tip: printing Green card front and back on the same page will save you a paper (as if you didn’t knew) and depending on the Notary public (like me!) you may save $$ on Notarization as well 😉
when you Don’t have an approved visa : You need to submit two visas, current visa and Previous visa. Visa receipt of Current visa along with last expired or expiring (previous) Visa, Both should be notarized.
Example: Recently you applied for Minor’s H4 visa and you have received just the receipt of Minor’s H4 visa extension (I-797C) , here you have to get notarized and submit minors H4 receipt (current visa) + Minor’s previous visa which will be I-797A (previously approved visa) or same previous visa which is stamped on minor’s passport.
3. Color Copy of Proof of Address
The Document checklist on VFS has mentioned a list of acceptable documents proof of address.
If minor have a current (means you residing at same address currently) and Valid (not expired) State ID or learning Driver’s License then well and good. But minor don’t have one you can submit one from the list in the document checklist like most recent Utility bill.
In other cases of applicant residing with parents, a Notarized undertaking (letter formally making a promise or pledge) from them with one of the notarized copies of their address proof should be submitted.
4. Annexure D
You can download this document from the checklist or from VFS site.
Already mention on document checklist – Must be signed by both parents and notarized.
If one of the parents is out of the country, please provide Annexure D from that parent duly notarized by the notary public in that country and a notarized No Objection Letter from the parent who is out of the country
Here’s what you fill in _ I/we <full father name & Full mother name as per minor’s passport> resident of <Complete USA address as per proof of address>…… <Full name of minor> and again <full father name > and <Full mother name>
Particulars of minor child
Name : <Name of minor as per passport or new name if Name change >
Date of birth : <DD/MMM/YYY>
Place of birth :<Exactly as per Passport>
Don’t forget to mention Passport No. for mother and father below your signatures.
Leave mother and father signature and date and place to fill in Infront of Notary Public.

5 & 6. Both Parents Valid Legal Status in the USA
If you have a Green card, EAD, or Stamped Visa on your passport like H1B, H4, get a color copy and front and back both side on same page for Green card / EAD.
When you Don’t have an approved visa : You need to submit two visas, current visa and Previous visa. Visa receipt of Current visa along with last expired or expiring (previous) Visa, Both should be notarized.
Example: Recently you applied for your H1B visa and you have received just the receipt of your H1B visa extension (I-797C) , here you have to get notarized and submit your H1B receipt (current visa) + your previous visa which will be your I-797A your previously approved visa or same previous visa which is stamped on your passport.
CONDITION BASED SUPPORTING DOCUMENT
1. Name change affidavit
You can find the Affidavit of name change document in the VFS Document checklist. TWO witnesses are necessary for Notarizing Affidavit of name change. Witnesses and Signer present before a Notary Public and sign the Affidavit.
On the first page find instructions on other documents to submit along with this Affidavit of Name change. If you still have a query, contact VFS even before starting application.

2. Annexure C
(Only required for Cases where One parent has not given Consent)

No, Vfs do not notarize the document.
You have to find a local notary person. I am notary myself, just in case you are from Dallas TX or nearby.
