How to fill out Annexure F
If your Indian passport is damaged due to issues like torn binding, exposure to rain, or drawings made by children, or if you have lost your passport and need a new one, you must complete the Annexure F form.
You can find Annexure F on your document checklist for damaged passports or on the VFS Global website.
Are Handwritten Affidavits Accepted?
According to VFS instructions, it’s recommended to type out the information rather than sending handwritten documents. However, I’ve personally seen hundreds of annexures and affidavits filled out by hand with blue pen, and they were accepted, including my own when I renewed my passport in October 2023.

EXAMPLE:
I [RAHUL MAHENDRA KUMAR] S/o, D/o, W/o Shri [MAHENDRA AMAR KUMAR]
residing at [1111 XYZ DRIVE, APT 222, CITY, NY, 77777]. solemnly affirm as follows:
- Begin by filling out the applicant’s full name, father’s name or husband’s name, and your current USA address.
EXAMPLE:
1.State how and when the passport was lost/ damaged and when FIR was lodged at
which Police Station and how many passports were lost/ damaged earlier?
LOST: While shopping, my documents were stolen from my car. Filed a police complaint on February 15, 2024, at ABCD station, with FIR no. A1235.
or
DAMAGED: Due to normal wear and tear
- If your passport is lost, provide as much detail as possible about the circumstances in 2-3 sentences.
- For damaged passports: Due to normal wear and tear or accidental drawings by my child.
EXAMPLE:
2.State whether you travelled on the lost/ damaged passport, if so state flight number
and date and port of entry into India?
NO
- In most cases, individuals don’t travel with a damaged passport, and a simple NO is sufficient. However, if you have traveled, feel free to provide the details.
EXAMPLE:
3.State whether you availed of any TR concessions/FTs allowance and if so details
thereof?
NO
- In most cases, individuals may not have received any travel-related allowances, so a simple “Not Applicable” or “NO” is acceptable.

For question number 4 : those in India who haven’t traveled outside the country, simply mark “NA”. However, if you are residing outside India, here’s how you fill it out:
- Start with the first time you left India with the current passport, for example, to the USA. If you arrived in the USA on January 22, 2018, and stayed for a few years before leaving on February 13, 2022, note the page number where your entry and exit stamps are.
- If you have traveled to the USA multiple times in the last 10 years, you should list all the trips you took to the USA as well as any other countries visited during that time period.
- If you’ve traveled to more than four countries and don’t have enough space to list all of them, add a new blank page and replicate the same columns as in question 4 to fill out the details for each country. In question 4 of this annexure, you can write “See attached page.”
- If you’ve lost your passport and don’t have a copy to fill out the details, you can simply provide the approximate information you have and mention, “Don’t know exact information as lost my passport.”
EXAMPLE:
6.State whether you were deported at any time at the expenses of the Government and
if so was the expenditure incurred reimbursed to Government of India.
NO
- In most cases, you may not have objections from the Passport Issuing Authority, so simply say NO or NA. If there were objections, provide the details.
- In most cases, the answer to this question, if you were deported or reimbursed expenses with government expense, will be NO or NA. If you have received any reimbursement or were deported, provide details.
Go ahead and make your signature, indicating the place and time of signing this annexure F.
