How to fill our Annexure E for Indian Passport Renewal

This Annexure E is a mandatory document required for both adult and child Indian passport renewals, to be submitted along with all other application materials.

Are Handwritten Annexure Accepted?

As per VFS instructions, it’s recommended to type out the information if you can use a PDF editor to fit in details. However, I’ve personally seen hundreds of Annexures filled out by hand with pen because people could not fit their long names or dates of birth in the space provided. VFS has accepted handwritten annexures, including my own when I renewed my passport in October 2023.

Annexure E for child

If you are completing this Annexure E for a child below 18 years, the applicant is the child. The child must sign (or provide a thumbprint if below 5 years old) for the applicant signature, and the only difference is that there won’t be spouse name. All other information remains as per the child’s passport.

What specific details are needed to fill in?

Begin by entering the applicant’s full name as per their passport (unless there are changes), followed by the father’s full name as per the passport.

For the residential address, ensure to provide your current US address. You need to submit one US address proof for the same address too.

Parents and Spouse name

As understood, you need to mention your father’s full name, mother’s full name, and spouse’s name (if applicable).

If you are adding your spouse’s name for the first time, you should submit a marriage certificate along with your spouse’s passport copy or any other ID.

All names should match your passport unless you require a change and have supporting documents for spelling changes.
